What the booking gathers

A callback number, the machine's address with its ZIP, which appliance is acting up, and the trouble told your way. That's the complete inventory — the form carries no hidden fields, and these pages run no advertising trackers.

Routing the visit, loading the van, and producing the written findings and the ticket. Records of finished jobs remain filed so warranty claims rest on real paper; the form itself rides one delivery relay and lands at the shop as ordinary email.
